**Chennai, November 3, 2024:** The Tamil Nadu government’s proposed transformation of the Institute of Mental Health (IMH) into a not-for-profit company has ignited controversy, raising questions about the state’s commitment to mental healthcare.

The proposal, outlined in two internal communiqués by Health Secretary Supriya Sahu, aims to modernize the 230-year-old institution and raise funds from national, international, and corporate sources. It suggests establishing a Section 8 company to manage IMH, arguing for greater financial and administrative flexibility.

However, the move has been met with strong opposition from professional doctors’ bodies who fear it signifies the state abdicating its responsibility towards its patients. Critics argue that while they welcome donations, the government should take responsibility for identifying and rectifying deficiencies within the institution.

The Health Department insists the proposal is an opportunity to clarify roles and enhance mental healthcare, ensuring treatment, human rights, and psychosocial rehabilitation. They cite the success of the Tamil Nadu Medical Services Corporation Limited (TNMSC), which has streamlined drug procurement and distribution, as a model for IMH’s proposed structure.

Despite the controversy, the government has repeatedly clarified that there are no plans to privatize IMH or hand over its management to an NGO.

Meanwhile, experts and organizations working in mental healthcare have acknowledged the successes of the District Mental Health Programme (DMHP) in expanding access to services, particularly in rural areas. However, concerns remain about the lack of sufficient psychiatrists, limited employment opportunities for those with mental health issues, and the need for increased focus on rehabilitation and social integration.

As Tamil Nadu strives to enhance mental healthcare, the debate over the proposed reforms highlights the need for a balanced approach that ensures the state remains accountable while leveraging resources and expertise from various stakeholders.
