According to eyewitness accounts from passengers Sabir, a Mumbai taxi driver, and Rajiv Sharma from Lucknow, a rumor of a fire in a general coach sparked chaos. Passengers, including women and children, jumped off the train onto the tracks. Before they could escape, an oncoming Karnataka Express struck them, resulting in multiple fatalities. Sharma reported yelling a warning about the approaching train but being unheard in the ensuing panic.
Railway officials suggest the initial spark or smoke in the Pushpak Express’s general coach may have been caused by a hot axle or brake failure. However, the ensuing stampede and resulting collision with the Karnataka Express are the primary cause of the deaths.
